How to Obtain Your Medical Record

If you need a copy of your Medical Record for services rendered during your hospital stay, please call 717-763-2660 or 717-763-2659. 

After calling, you may download the Authorization for Release Form (pdf). Please complete the form, then return it to Health Information Services,
